PacifiCare Health Plans- A United Health Insurance Company
PacifiCare Health Systems is one of the largest health care services companies in the United States. PacifiCare Health Systems, a fortune 500 company headquartered in California, serves more than 3 million health plan members and approximately 10 million specialty plan members nationwide with annual revenues of more than $12 billion. PacifiCare Health Systems was acquired by United Health Group in late 2005, which however continues to market health plans under the original PacifiCare name.

PacifiCare SignatureValueSM HMO: This plan stresses on the quality of healthcare provided to each individual member aimed at prevention of illnesses and general health upkeep. Clients can directly contact the primary care physicians (PCPs), and build good rapport with their doctors. Members pay a small, predetermined co-payment each time they receive health care services. This plan offers a network of over 47,000 physicians and 222 hospitals.
PacifiCare SignaturePOSSM POS: This Point-of-Service plan provides optimum flexibility to PacifiCare's clients and offers HMO savings coupled with the option to adopt either an in-network or out-of-network model to healthcare.
PacifiCare SignatureOptionsSM PPO: These plans offer clients maximum freedom in choosing a physician, and customers may see any participating physician or specialist without any extra payment. Members are also at liberty to consult out-of-network doctors, but will be charged.
PacifiCare SignatureIndependenceSM Indemnity: This is a standard Indemnity plan to allow members the freedom to consult any doctor/specialist without a referral. Indemnity plan members must pay an annual deductible for coverage.
PacifiCare SignatureFreedomSM SDHP: PacifiCare's Self-Directed Health Plans are PPO based to allow clients widest access to the physician network, and the freedom of a Self-Directed Account (SDA). This SDA may be used at the member's discretion, for preventative health services such as doctor's visits and covered exams. This plan offers a network of over 68,000 physicians and 330 hospitals.
Other specialty products of PacifiCare include behavioral health services, life and health insurance, dental and vision services and pharmacy benefit and medical management through its wholly owned subsidiary, Prescription Solutions.
