Nationwide and California Health Insurance:

If a person wants to apply for nationwide health insurance in California, he must first get membership of California Farm Bureau Federation (CFBF). CFBF is the largest organization for farmers, ranchers, business people and citizens in the United States. California Farm Bureau Federation was formed in 1919. Today it has nearly 95,000 members in 56 countries. It has provided affordable health insurance to its members since 1947. Both Nationwide Health plans and CFBF together have developed PPO plans for its individual and family plans.

Nationwide PPO Choice Plans:

• Nationwide PPO Choice Saver Select

These are HSA compatible, high-deductible, PPO plans. If you are an individual, you can select a deductible amount of $1,650, $2,400, $3,600 or $4,800. For families, the deductible amount would be double the individual. As the deductible amount increases, monthly premium will decrease. If your expenses exceed $25, you have to pay for out-of-network visits. Maternity benefits are only provided on the two higher deductible options only.

• Nationwide PPO Choice 30 Plan

Three deductible options under choice 30 plan category are $750, $1500 or $2500. Physician visits and preventive health care services are provided at a set co-payment of $30 and are not subject to your deductible. Maternity coverage is included in this plan.
