Basic
PPO 1000/2500: This plan is available at affordable prices and
you can either choose a $1000 or $2500 annual medical deductible. It offers
coverage for hospitalization and emergency services. These plans do not
include maternity coverage or prescription drug benefits.
PPO Share Plans: These plans offer wide range of benefits with a flexibility to choose a doctor and hospital of your choice. You can see difference in deductibles, coinsurance amounts and annual out-of-pockets.
PPO Saver Plans: This plan offers benefits like hospitalization, emergency services and prescription drugs. You can avail more benefits after you meet annual deductible/annual out-of-pocket maximum requirements.
Right Plan PPO 40 and 3500 Deductible PPO plans are set up for young adults and early retirees. For 3500 Deductible PPO plans you just have to pay annual deductible and zero dollars for in-network services.
HMO Plans: Anthem Blue cross blue shield insurance of California offers three HMO plans. The Select HMO, HMO Saver and Individual HMO plans.
Lumenos Plans: Lumenos plans are available in three types. Lumenos HSA Compatible plans, Lumenos HIA plan and Lumenos HIA plus Plans.
Tonik Health Plans: Tonik offers three simple plans to choose from with rates as low as $64 - $123 per month, depending on the plan you pick and where you live.
